The "Co-Production in Palliative Care; Exploring Integrative Models' Conference is taking place on the 25 January 2017 in the White Horse Best Western Hotel, Derry/Londonderry.
The half day conference will explore three models of care, funded by the Integrated Care Partnership, being delivered to patients, carers and health professionals in the Western area. We hope to explore how we might best develop and expand these models to meet the combined challenges of geography, demographics and resources in the Western area. Foyle Hospice, Western Health & Social Care Trust, Integrated Care Partnership are the three conference partners.
The conference is free to attend and lunch is provided.
