
The IIOP is delighted to announce the availability of a new resource on the subject of Open Disclosure. This programme, aimed at pharmacists practicing in the Irish setting consists of two modules:
Module 1 (30 minutes) covers the principles of open disclosure and the professional, regulatory, legal and ethical obligations in relation to it. It is based on the National Open Disclosure Framework by the National Patient Safety Office. The topics include:

Module 2 (45 minutes) covers the open disclosure process and documentation requirements following patient incidents involving no harm and minor, moderate and higher levels of harm. The topics include:

Topic 2: Managing low level open disclosure

Topic 3: Managing high level open disclosure

Topic 4: Open disclosure of a notifiable incident (Patient Safety Act 2023)
At the end of this programme learners will be able to:

Recognise their professional, regulatory, legal and ethical obligations in relation to open disclosure.

Identify what open disclosure means, when it is required and why it is important.

List the principles of the National Open Disclosure Framework 2023.

Access additional resources available to support you when implementing and engaging in open disclosure in your practice.

Identify ways to embed a ‘just culture’ within pharmacy teams.

Identify the correct communication strategies for effective open disclosure using the ASSIST Model.

Identify the ten principles that underpin the open disclosure process.

Manage the open disclosure process and documentation requirements for patient incidents involving no harm and minor, moderate and higher levels of harm.

Identify the various staff roles involved in open disclosure and how these roles support the patient, family and staff affected by patient safety incidents.

NOTE: For pharmacists working in HSE and HSE funded services – this programme does not replace HSE mandatory training requirements but is supplementary and developed specifically by IIOP for pharmacists.
