This programme, aimed at pharmacists practicing in the Irish setting consists of two modules:
Module 1 (30 minutes) covers the principles of open disclosure and the professional, regulatory, legal and ethical obligations in relation to it. It is based on the National Open Disclosure Framework by the National Patient Safety Office.
Module 2 (45 minutes) covers the open disclosure process and documentation requirements following patient incidents involving no harm and minor, moderate and higher levels of harm. The topics include:
NOTE: For pharmacists working in HSE and HSE funded services – this programme does not replace HSE mandatory training requirements but is supplementary and developed specifically by IIOP for pharmacists.
