X-PERT is a FREE structured dietetic care and diabetes patient education programme for adults with Type 2 Diabetes. It is an award winning evidence based programme provided by the HSE Community Dietitians in Cork & Kerry. It is open to all people with type 2 diabetes – by self referral or via a health professional.

Who is this training aimed at?              

Pharmacists  - in response to requests

Attending this training will?

  •  support you in providing integrated care to your clients with type 2 diabetes
  •  ensure you are fully informed of current HSE Community Dietitian services and education courses for your clients to assist them in accessing services
  •  inform you of how X-PERT encourages clients to connect with their local pharmacy to support their diabetes care
  •  increase your knowledge of current diet and lifestyle treatment of type 2 diabetes
  •  support sharing of experiences of pharmacists in supporting type 2 diabetes
  •  provide a CPD opportunity with your colleagues

What will the training cover?

It will be an interactive workshop using the X-PERT visual aids and resources, covering:

  •  Challenges for those living with type 2 diabetes
  •  Demystifying ‘what is diabetes’ for your client
  •  How lifestyle factors affect type 2 diabetes care
  •  Empowering people to self-manage their diabetes
  •  Diabetes health results – supporting clients to make changes
  •  How to refer to Dietitian services for type 2 diabetes and details of upcoming courses

Date & Venue: Wednesday 15th November  in the SMA Parish Centre, Wilton (Beside the church)

Duration:  7.00pm-9:00pm  (2hours)                                                                                

The training is FREE of charge & is facilitated by HSE Community Dietitians.
